Nominating a Collins Pharmacy in England and Wales for Prescription Collection

The NHS Electronic Prescription Service (EPS) allows most GPs in England and some in Wales to send prescriptions electronically to a nominated pharmacy of your choice. With EPS, there's no paper prescription, so you won't have to worry about keeping track of bits of paper.

By completing the form below, you can nominate a pharmacy to collect your prescriptions. This is ideal if you'd like to choose a Boots pharmacy for prescription collection or if you're nominating someone else.

All fields are required unless marked otherwise.

How do I nominate or switch to a Collins Pharmacy in Northern Ireland & Scotland?

The Electronic Prescription Service (EPS) does not exist in Northern Ireland and Scotland, but don’t worry other options are available.

Find the most convenient store to take & collect your prescription. This may be the closest store to your home or work, or check out where our midnight pharmacies or pharmacies with extended opening hours are. Whatever is most convenient for you. You can also check pharmacy stock online before going into store.

You can ask your GP surgery to make a note of the pharmacy of your choice. If you are in Scotland, speak to your surgery about the Medicines Care & Review Service where you can register with a pharmacy of your choice.

How do I nominate or switch to a Collins Pharmacy in Wales?

The Electronic Prescription Service (EPS) is currently being rolled out in Wales, with a large proportion of our stores able to accept electronic prescriptions and are working with GP surgeries. Your GP surgery will let you know when they will be able to send prescriptions electronically to your preferred Pharmacy. If you still receive paper prescriptions from your GP simply take these to any Collins Pharmacy. If your GP is using EPS, you can simply nominate a Collins of your choice using this form.

Find the most convenient store you would like your prescription to be sent to, (or where you can take your paper prescription if your GP surgery is not yet able to send it electronically).
